North West Transition Network Event

The conference aims to bring together young people aged 13-25 and senior health professionals working in this area, to explore transition issues and identify positive strategies for change. The conference programme will be designed and run by young people from the Young Ministers of Health Project and we are currently looking for other groups of young people to take part in the day.
The day will provide opportunities to identify debate and explore solutions to issues affecting young people as they move from children’s to adult services. Workshops will be designed and facilitated by young people.
