The Transition Information Network (TIN) is hosted by the Council for Disabled Children. The Council for Disabled Children is part of a partnership that is running the government’s new transition programme; Preparing for Adulthood.

The Transition Information Network (TIN) is an alliance of organisations and individuals who come together with one common aim; to improve disabled young people's experience of transition to adulthood.

TIN provides information about transition through its website, magazinee-bulletin and seminars. It is free to become a member of TIN.
