Calderdale have just completed the second part of the pilot plan to introduce person centred transition reviews into Calderdale mainstream schools and educational settings.

Valuing People Guidance on Person Centred Approaches was published in 2002. It was written to help Learning Disability Partnership Boards work out how they could write 'frameworks for person centred planning'.
