The purpose of the Transition Information Network is to provide information about the transition process in England for disabled young people in transition to adulthood. The main way we provide this information is through our website. It signposts to relevant information about transition and organisations that provide information, advice and support. On the left hand side you can find resources covering a wide variety of topics.

The Transition Information Network has also written the following key resources on transition:

  • A Transition Guide for All Services (DCSF, 2007)
    This guide brings together all key information for professionals about the transition process. It includes information on statutory duties and offers examples of effective practice to illustrate how services are addressing local need. It contains checklists, which offer information on meeting key obligations and providing minimum quality standards, to support agencies in providing a framework of services to effectively meet the needs of disabled young people in their transition to adulthood. This guide is a starting point to give all services useful information and examples of excellent support for disabled young people and their families. There are no more hard copies available but the guide can be downloaded.
  • TransMap: From theory into practice (NCB, 2009)
    The Council for Disabled Children was funded by the Department for Children, Schools and Families to map effective practice in the improvement of transition processes, and to identify some of the barriers that local areas face. Building on the good practice identified for the Transition Guide for all Services, the TransMap project set out to identify areas of effective practice and to provide evidence of current practice and developments in transition. The launch of the Transition Support Programme in 2008 and other targeted policy and guidance has encouraged many local areas to prioritise the transition process, and to identify some key areas to focus on. This report highlights the impact of these developments and documents the key findings from the TransMap project. The TransMap work identified a number of principles that, when applied, lead to a high quality service for young people in transition. These are:
  • comprehensive multi-agency engagement
  • the full participation of young people and their families
  • the provision of high quality information
  • effective transition planning
  • an array of opportunities for livining life
