DESCRIPTION: The post holder will be required to review the transition assessments/plans for identified young service users, aged 14 – 25 years old, coming through to Adult Services from Children’s Services, with the view of assisting them to move to living independently in the community either with or without support. Working with service users, families, carers and service providers will be a crucial part of this work.

SALARY: £25,822 p.a.

LOCATION: Walthamstow, East London.
CONTRACT TYPE: One year with potential further pending funding
HOURS: 37pw

QUALIFICATION LEVEL: Diploma in Social work or similar qualification (e.g. NVQ Level 3 or above) DESIRABLE
