Transition survey: Epilepsy services
Young Epilepsy (formerly known as The National Centre for Young
People with Epilepsy) is a charity that works on behalf of people
aged 25 or younger with epilepsy.
Young people and their families have informed us that they face
a ‘cliff edge’ where services fall away when the young person
reaches the age of 16 or leaves school. We are hoping to
develop a best practice model for transition to help young people
move into adulthood.
We have asked DJS Research, an independent research company, to
carry out a survey to find out more about what services people
would like to see available through the clinics.
The survey is kindly being funded by GlaxoSmithKline.
We would really appreciate it if you could take a few minutes to
complete our on-line questionnaire. Your feedback will help to
inform the design of the model.

We really appreciate you taking the time to complete this
survey.
Many thanks
