Welcome to the Transition Information Network

This is a website for people who support disabled young people in transition to adulthood. The aim of this website is to provide information about the transition process that will be useful to professionals and parents/carers. There is also a section for young people that talks about the transition process and gives links to helpful resources.

TIN has recently developed an online Resource Library. This was created to 'house' the resources that were originally produced by the National Transition Support Team and partners, Child Health and Maternity Partnership (CHaMP) and National Strategies (NS), for the Transition Support Programme (TSP). The TSP (part of Aiming High for Disabled Children) was a 3-year government programme that aimed to raise the standards of provision for disabled young people in all local areas in England.
